Social Work Student Intern – PPHC
FUNCTION(S): Provide consumer related support to the Case Management Unit and staff.
RESPONSIBILITIES:
Students completing the field education requirement for an Associates Degree in Social Services Technology, a Bachelors or Masters Degree in Social Work may complete the following:
- Monitor Medicaid eligibility of PASSPORT Home Care (PPHC) enrollees.
- Serve as a liaison to County Departments of Job and Family Services related to Medicaid eligibility and Financial Liabilities.
- Perform data entry of consumer information into the PASSPORT Information Management System (PIMS).
- Retrieve voicemail for absent Case Managers, as assigned.
- Handle incoming consumer, provider, and other communication requests for absent Case Managers, as assigned.
- Participate in and contribute towards accomplishment of divisional and Agency goals.
- Perform other tasks as assigned by Field Education Instructor, Supervisor or Case Management Mentor.
Students completing the field education requirement for a Bachelors or Masters Degree in Social Work may complete the following under the direct Supervision of a Licensed Social Worker:
- Evaluate assessment and case management data to determine ongoing program eligibility, care needs and service options.
- Monitor and evaluate the care plan, service quality and program eligibility.
- Complete the enrollment process of eligible enrollees into the PPHC program.
- Provide education and support to PPHC enrollees and their caregivers.
- Advocate on behalf of consumer and/or caregiver/family and assist individuals in securing appropriate services and care settings.
- Disenroll all ineligible enrollees from the PPHC program.
- Apply judgments and perform clinical practice based upon education, experience and professional practice standards.
AUTHORITY AND RELATIONSHIPS: Responsible to Field Education Instructor, Licensed Social Worker (LSW) Case Management Supervisor and/or LSW Case Management Mentor.
POSITION QUALIFICATIONS: Must be actively pursuing an Associates Degree in Social Work Technology and/or a Bachelors or Masters Degree in Social Work through an accredited university.
PHYSICAL/MENTAL POSITION QUALIFICATIONS: This position requires its occupant to have the following abilities to successfully perform the essential functions of the job.
- Ability to effectively communicate with the elderly population and the general public, both in person and by phone,
- Ability to comprehend written material related to essential job functions,
- Ability to accurately document facts related to essential job functions,
- Ability to comprehend, recall and apply facts related to essential job functions,
- Ability to analyze, evaluate, and implement a reasonable course of actions based on available information,
- Ability to detect specific odors related to the assessment of a consumer’s health/hygiene and the safety of the consumer’s environment,
- Ability to legally drive a car or make other transportation arrangements, and
- Ability to ambulate enough to access private homes/locations which may not readily be accessible.

Community Outreach Intern
Function(s): Provide support to the Agency’s community outreach and health education staff.
Scheduling: This internship will have flexible scheduling, and shall not exceed twenty-five (25) hours of work per week. Work schedules will be based on event and speaking engagement availability and office hours to be held by the intern.
Responsibilities:
- Provide staff support at community speaking engagements and health fairs
- Participate in the Agency’s Speaker’s Bureau
- Assist with coordinating education forums and events
- Help coordinate the creation and distribution of communication materials among internal and external stakeholders
- Assist with the outreach of fundraising activities
- Monitor the Agency’s website for accurate, up-to-date information
- Assist in development of advocacy tools and resources relating to state and federal elected officials
- Assist in development and promotion of health education materials
- Perform other tasks as assigned by supervisor
Authority and Relationships: Responsible to Agency’s Community Outreach Manager.
Compensation: This is a volunteer position.
