In-Home Care
Older adults sometimes need help to remain in their home safely. Help can come in many forms, from a person who helps with cleaning the house to someplace to take a loved one for the day so you can have a break.
The three options for in-home help are:
- PASSPORT - A Medicaid program designed to keep older adults at home instead of a long term care facility through a range of supportive services.
- Care Coordination - A program designed to assist nearly indigent and physically frail older adults with in home services to keep them as independent as possible.
- Finding Home Care Providers - Private programs for older adults who do not qualify for the above programs mentioned.
What kind of help do you need?
The PASSPORT program and our Care Coordination program use the following services to help keep older adults at home.
- Adult Day Care: An out-of-home program of structured activities in a group setting. Transportation (if necessary) is provided.
- Chore: Major housekeeping tasks such as cleaning windows, walls and carpets, as well as pest control.
- Homemaker: Assistance with shopping, meal planning and preparation, and light housekeeping.
- Home Delivered Meals: Hot or frozen meals delivered to an older person's home.
- Minor Home Modification: Home improvements and repairs related to safety or accessibility.
- Personal Care: Assistance with bathing, dressing, grooming and toileting.
- Safety Monitoring: An electronic unit that alerts family, friends or caregivers if the older person needs help.
