Care Coordination Program

The Care Coordination Program provides help to older adults who want to remain independent and living in the community. Enrollees in this popular program receive personalized services that are coordinated by a care manager. The case manager can assist with things like disease management, health insurance benefits, service coordination and transitional care. Our professionally trained Care Coordination staff have experience working with families who have loved ones with chronic conditions, such as heart disease, cancer or Alzheimer's disease. Our specialized staff can:

  • Provide information and assistance in all aspects of the aging process
  • Provide services, such as homemaking or personal care
  • Provide an emergency response system
  • Offer trainings on age-related topics
  • Support caregivers (spouses, children, neighbors)
  • Help you or your loved one remain independent
