Assisted Living Medicaid Waiver Program

The Assisted Living Medicaid Waiver program is designed to promote independence and improve physical and social well being by providing participants individualized care and assistance with housing, personal care and heath care services.

Eligibility

To be eligible for Assisted Living you must be:

  • Residing in a nursing facility on Medicaid, on a Medicaid Waiver Program (PASSPORT, Choices, Ohio Home Care or Transitions Carve Out) or residing in a Residential Care Facility for at least 6 months immediately prior to the date of application for the Assisted Living Waiver Program
  • 21 years of age or older
  • Able to pay a monthly room and board payment of $587

You must also be at an Intermediate or Skilled Level of Care which means you require hands-on assistance with at least two of the following:

  • Mobility/Transferring
  • Bathing
  • Toileting/Using the Restroom
  • Grooming
  • Eating
  • Taking Medicines

Services

Assisted Living Services

  • 24-hour access to staff that will manage the scheduled and/or unscheduled needs of the enrollee
  • Personal care
  • Housekeeping, laundry and maintenance
  • Three meals a day
  • Non-medical transportation
  • Social and recreational programs
  • Nursing service (health assessment monitoring and medication oversight)

Community Transition Service Funding

  • Funds available only to enrollees leaving the nursing facility and relocating to the Assisted Living Facility that are used to purchase household furnishing supplies and incidentals not provided by the facility, formal/informal supports
  • Will not exceed $1500 per enrollee and can only be used for items that are essential to the enrollees health and welfare
  • Must be used within 90 days of enrollment and cannot be used for recreational purchases

Q: What is the reimbursement an Assisted Living Facility will receive after they accept an enrollee?

A: The reimbursement rate a facility receives includes the room and board payment of $573/month (paid by the enrollee) and a daily rate for services (paid by the program) that is based on the level of service an enrollee needs.

Tier 1 $49.98/day $2086.40 ($587 + $1499.40)
Tier 2 $60/day $2387 ($587 + $1800)
Tier 3 $69.98/day $2686.40 ($587 + $2099.40)

Q: How much income is an Assisted Living Program enrollee entitled to keep monthly?

A: Each Assisted Living enrollee is entitled to keep $50/month as a personal needs allowance.
